There is a Dual Wield powerup available in this room, and once the enemies are dead, the skull is free for the taking. Destroying one group will spawn a Malicious Face and a Virtue. The blue skull is guarded by several Schisms and Soldiers, along with two Stalkers spawning close by and drones for air support.
